Your Geriatric Care Manager: - Conducts a comprehensive needs assessment to identify current and future problems and provide practical solutions.
- Screens, arranges and monitors appropriate in-home services on an on-going basis.
- Acts as a liaison to families, overseeing care and quickly alerting family members of any problems.
- Assists with moving a loved one, if needed, to or from a retirement community, assisted living residence or nursing care facility.
- Provides education, advocacy, guidance, counseling and support to your loved one.
- Facilitates and guides family/caregiver consultation meetings regarding a loved one’s care needs.
